MINISTERIAL APPLICATION ORDER FORM
The word Ordination is from the Latin ordinare, meaning "to set in order." Ordination is a ceremonial service in which a person is set aside, ordained for ministry, or special service within the "Body of Christ."
The cost of the Evangelical Ministerial Associationtm application form booklet is $18.00. This includes the shipping and handling cost. The booklet consists of the complete "Declarative Statement" and the "Application" form.
The candidate for credentialing will receive the booklet which explains in detail the basis on which the credentials will be granted. Following the submittal of the application to the Evangelical Ministerial Associationtm, the Credentials Committee will evaluate this information.
Inasmuch as the Evangelical Ministerial Associationtm is a ministry department of the Evangelical Theological Seminarytm, the association booklet and application can be received by contacting the seminary. This can be done by contacting the association through the seminary, by e-mail, telephone, or a written request. The Ministerial Application Order Form can also be sent to the interested candidate by e-mail. The $18.00 payment for the booklet can be made by money order, cashier check, personal check, or credit card.
